Post Discharge: This program makes phone outreaches to Medicare members who are discharged from the Emergency Department/Emergency Room (ED/ER). The Care Team will make outreaches to these members and offer a no-cost telephonic consultation with a Call Center Representative. Concerns that may be addressed may include, but are not limited to, issues surrounding the member\u0027s understanding of diagnoses, providing resources for hot meals, transportation, and local urgent care or Telehealth options, and making other recommendations for resources that may benefit their care, and discuss other follow-up needs.
