Under the supervision of a Pharmacist Team Lead and Head of Operations, you will be responsible for making outbound calls to patients, providers, and pharmacies to intervene in patient cases identified using risk stratification algorithms. These interventions may involve obtaining and recording health and contact information, scheduling appointments, resolving medication-related issues, and performing patient engagement activities. In addition to patient interactions, you will be responsible for answering inbound customer support requests, preparing, and mailing letters, and other assigned duties.
