Coordinate and Manage Patient CareServe as the primary case manager for assigned patients, coordinating all care services, community resources, and medical appointments.Facilitate communication between all parties involved in care?including physicians, therapists, and caregivers?to ensure smooth continuity.Ensure adherence to state, federal, and agency policies.Complete supervisory visits for LPNs (every 60 days) and HHAs (every 14 days).Lead the full case management and care-plan development process for new admissions.3. Ensure Quality, Safety, and Regulatory ComplianceMaintain accurate, timely documentation in alignment with regulations and internal policies.Follow professional standards of practice, including providing direct patient care when required.Monitor and coordinate care across all disciplines within assigned caseloads.Participate in quality improvement efforts, evaluating clinical data to identify trends and areas for improvement.4. Exact compensation may vary based on several factors, including skills, experience, and education.
