These Nurses will operate on a hybrid model (3 days in person patient visits, 2 days remote patient... More intakes). They will be responsible for conducting initial assessments and determining qualification for long-term home care. Assessments of the patient and their environment will be thorough, lasting 2-2.5 hours each. For the first ~2 weeks, RNs will attend remote training sessions via Zoom from 8:30-5 PM. From there, RNs will be paired with Preceptors and will observe them conducting assessments for 1-2 days.
