This person will be responsible for:?Planning, implementing, coordinating, evaluating and promoting... the continuity of care consistent with established standards and practices. ?Assessing patient/family needs and providing teaching and other instructional material.?Preparing and administering therapies under the direction and supervision of a physician.?Administering all medications/blood products using correct medication/blood product, dosage/amount, route, time and technique. ?Assuring pre-authorization and appropriate consent forms are obtained prior to administration of therapies where applicable.?Facilitating dose modification by assessing patient, assessing labs, consulting protocol or treatment plan, calculating dose, and consulting physician regarding dosage modifications, problems and symptom management ?Performing venipuncture, accessing implanted ports and maintaining central lines catheter devices.?Maintaining accurate documentation of assigned patient care and treatments provided.?Applying knowledge of reimbursement procedures and charge capture.We are a company committed to creating diverse and inclusive environments where people can bring their full, authentic selves to work every day.
