Insight Global is seeking a compassionate and detail-oriented Care Manager in Woodland Hills, CA to support patients enrolled in our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM)... programs. This role plays a critical part in coordinating care, maintaining consistent patient engagement, and ensuring patients receive the guidance and support needed to effectively manage chronic conditions. The ideal candidate is patient-focused, organized, and comfortable working in a remote, technology-driven healthcare environment.In this role, you will spend your day engaging directly with patients via phone to provide ongoing support, education, and care coordination as part of our CCM and RPM programs. You will develop and maintain individualized care plans, monitor patient vitals and compliance data, and document all interactions accurately.
