Insight Global is looking for a Licensed Practical Nurse (LPN) that participates in providing patient care for one of our largest healthcare providers. The ideal resource will be responsible for the below duties:
-Outreach and scheduling for patients post hospitalization
-Maintaining accurate records of all outreaches
-Location and upload of discharge summaries
-Providing patient education on Medication Adherence
-Medication reconciliation and review
-Datamining/charting
-Educating providers about the appropriate quality measures
NOTE: Travel is required for this role. 60% of the week is in the office and 40% of the week is floating/traveling between clinics. Travel may require commuting as far as 20 miles outsides the Austin area. Mileage reimbursement WILL be provided.

-Current unrestricted Vocational Nurse License in applicable state
-Highschool diploma or GED
-Current BLS Certification (if not certified - must acquire certification within 30 days of hire)
-At least 2 years of LVN experience in a clinical setting
-Valid driver's license within the state of work
-Knowledge of medical terminology and basic computer literacy
-Proficient in Microsoft Excel and PowerPoint
-Ability to learn quickly and communicate effectively
-EMR experience
-Willingness to travel/commute
