Job Description
The Hospice LPN plans, coordinates, and delivers compassionate end‑of‑life care to patients and families within their homes and community settings. This role performs comprehensive assessments, develops and manages individualized plans of care, and provides skilled nursing services focused on comfort, symptom management, and quality of life. The LPN serves as a key member of the interdisciplinary hospice team, acting as a patient advocate and clinical resource. As assigned, the nurse functions as a Case Manager, coordinating services and supervising hospice aides to ensure continuity of care. The position requires strong clinical judgment, communication skills, and the ability to work with a team in a facility and in-home setting.
Key Responsibilities
Perform initial, ongoing, and comprehensive assessments of patients and families to determine hospice needs
Develop, implement, and regularly revise individualized plans of care using the nursing process
Provide skilled nursing care, including pain and symptom management, medication administration, and treatments per physician orders
Educate and counsel patients, families, and caregivers on disease process, self‑care, medications, and comfort measures
Act as Case Manager for assigned patients, coordinating care across physicians, interdisciplinary team members, and community resources
Maintain accurate, timely clinical documentation, including pain and symptom outcomes
Communicate patient status changes to physicians and obtain orders as needed
Supervise hospice aides and delegate care appropriately
Participate in interdisciplinary team meetings, on‑call rotation, and quality improvement initiatives
Ensure a safe care environment and arrange for necessary equipment, supplies, and services

Required Skills & Experience
Active LPN license in the state of practice (Rhode Island or Compact)
Current CPR certification
Valid driver’s license with reliable vehicle
Nice to Have Skills & Experience
At least 1 year of hospice nursing experience
Familiarity with hospice EMR systems and interdisciplinary care models
1–2 years of recent acute care nursing experience and 1–2 years of recent home care experience
