Job Description
We are seeking experienced Hospice Registered Nurses (RNs) to support an urgent staffing need in the Beachwood, OH area. This contract role focuses on terminally ill patient care primarily within assisted living and skilled nursing facilities. The role centers on daily visits, hospice admissions, symptom management, and close collaboration with families and interdisciplinary staff.
Responsibilities:
Provide direct nursing care to hospice patients in their place of residence.
Assess patient condition, develop, implement, and revise individualized plans of care.
Collaborate as an active member of the Interdisciplinary Team (IDT).
Demonstrate strong knowledge of hospice philosophy and end‑of‑life care principles.
Communicate compassionately and effectively with patients and their families.
Deliver professional nursing care using the nursing process and sound clinical judgment.
Identify and report changes in patient condition to physicians and care team members.
Complete accurate, timely clinical documentation per hospice and regulatory standards.
Participate in on‑call rotation, including evenings and weekends.
Supervise Hospice Aides and LPNs as assigned.
Educate patients, families, and caregivers on care needs and disease progression.
Adhere to confidentiality, quality, and safety standards and participate in QAPI initiatives.
