Job Description

A leading home healthcare provider is seeking a Clinical Nurse Manager to join their Elmsford, NY office. This is a full-time position offering a balanced mix of fieldwork and office-based responsibilities.
Fieldwork (40%)
o Conduct monthly visits for each case (caseload ranges from 15 to 30 clients)
o Support admissions and perform new client assessments
o Develop and update care plans
o Supervise and mentor field staff
o Territory includes northern Bronx and Westchester County, with occasional coverage in Putnam, Rockland, and Orange Counties
Office-Based Duties (60%)
o Observe and validate field staff competencies (new employees and annuals)
o Review and ensure accurate case load documentation
o Lead staff training sessions and in-service education

Required Skills & Experience

2 years of RN experience, must have experience working in Home Health

Excellent communication skills

Team player

Graduate of an accredited and approved nursing program; BSN preferred

Nice to Have Skills & Experience

Previous management experience

Home health care experience highly preferred
