Insight Global is seeking 81 Home Health Nurse Assessors to join a healthcare client in New York. These Nurses will operate on a hybrid model (3 days in person patient visits, 2 days remote patient intakes). They will be responsible for conducting initial assessments and determining qualification for long-term home care. Assessments of the patient and their environment will be thorough, lasting 2-2.5 hours each. For the first ~2 weeks, RNs will attend remote training sessions via Zoom from 8:30-5 PM. From there, RNs will be paired with Preceptors and will observe them conducting assessments for 1-2 days. The Preceptor will then observe the RN conducting assessments and provide reviews and feedback. Once training is complete, expectation will be 2 in person assessments during field days and 3 telehealth assessments during virtual days.

* 22 months of experience in a home health setting within the past 15 years (does not need to be experience providing long term care; any form of interfacing with a patient in the home will work)
* Some experience conducting patient intakes/assessments
* Computer literacy (Outlook, Zoom, MS Teams, screenshots, attachments, typing 35 words per minute)
* ASN or BSN
* BLS Certification from an accredited program
* Unencumbered NY State license
* Experience utilizing an online patient assessment system; Uniform Assessment System (UAS) preferred
* Knowledge of NYIAP program
