Job Description
Insight Global is looking for a case manager to join one of their healthcare clients within the New Jersey area. They will provide person-centered support to older adults living with dementia and their caregivers, ensuring continuity of care across medical, behavioral, and community resources. They will be conducting comprehensive assessments of cognitive, functional, and safety needs; developing individualized care plans; and coordinating services such as adult day programs, respite care, transportation, and home safety evaluations. They will collaborate closely with an interdisciplinary treatment team and family members through regular huddles and case conferences, while also engaging caregivers with education and coaching on disease progression and communication strategies. This role requires proactive problem-solving, strong organizational skills, and the ability to work independently while fostering teamwork. By taking initiative and leveraging community resources, you will help reduce care gaps, improve quality of life, and support both clients and families throughout the dementia care journey.

Required Skills & Experience
· Bachelors degree in Mental Health Field
· Experience with a geriatric population
· Experience with community resource for those impacted by Dementia
· Experience working collaboratively with the treatment team and family members
Works well independently and within a team
