Job Description

We are seeking a Temp RN to support AZ CSNP Transitions of Care program with transition of this insurance company's membership. This position supports the Regulatory and delegated programs for management of complex members, including comprehensive assessment and care planning. Works directly with the members in establishing self-management of their chronic illness.

Required Skills & Experience

Compact RN license in state of residence

Chronic Condition/Special Needs experience (diabetes, chronic heart failure/disease etc)

Case management experience
