Job Description
As an LPN Case Manager, you will be responsible for assessing, planning, implementing, reevaluating and revising care and coordinating resources for program participants. You will update and maintain patient records, implement orders for medications and treatments and perform assessments. You will also ensure ongoing education, training, and professional development of all staff that is relevant to care of program participants.

Required Skills & Experience
• Willingness to travel within assigned region required.
• Valid driver’s license and reliable transportation required.
• Fluency in both English and second language strongly preferred.
• Licensed as LPN by Massachusetts Board of Registration in Nursing.
• Minimum of one year experience in a skilled nursing or home care setting or comparable work experience preferred.
• Case management, behavioral health, social services and/or community health worker experience preferred.
• Strong communication, documentation, and organizational skills are required.
• Must be solution oriented and be able to multitask.
• CPR Certification.
