Job Description
Registered Nurse Care Coordinator supports patients after they leave the hospital or a short‑term rehabilitation facility. The focus is on keeping patients safe, stable, and connected to the right medical care as they recover at home.
You work closely with physicians, primary care teams, specialists, home health, palliative care, social work, and community partners to help patients avoid complications and prevent unnecessary readmissions.
Overall purpose: Improve patient outcomes, reduce avoidable readmissions, and support long‑term wellness through ongoing, relationship‑centered clinical care.
