Insight Global is looking for a Clinical Coordinator supporting the Acute Rehab center at a hospital center in Washington, DC. We are seeking a dedicated and knowledgeable Clinical Liaison to serve as a key connector between internal and external referral sources and the inpatient rehabilitation program. This role is essential in ensuring that patients who can benefit from inpatient rehab services are identified, assessed, and admitted in a timely and clinically appropriate manner.
Key Responsibilities:
Act as the primary point of contact for referrals to the inpatient rehabilitation unit, fostering strong relationships with physicians, case managers, and other healthcare professionals.
Conduct thorough clinical assessments and screenings to determine patient eligibility for inpatient rehabilitation, in collaboration with the rehab physicians and Medical Director.
Coordinate and facilitate the admission process, ensuring a smooth and responsive transition for patients entering the rehab program.
Work closely with hospital case management teams to streamline communication and optimize the referral workflow.
Serve as a subject matter expert in inpatient rehabilitation, educating referral sources and stakeholders on program capabilities and patient criteria.

Current license to practice as RN or RN case management in DC
Case management experience
1 year of acute rehab experience preferred
