One of Insight Global's largest home health agency clients is looking for a Patient Care Manager in Aurora, CO. The Patient Care Manager is someone who has home health and people management experience. This person will work full-time on a competitive base salary with a quarterly bonus structure, and act as a support function to the clinicians in the field treating patients in short-term acute homecare. The incumbent will answer questions from the field, go on ride-alongs, review documentation, train employees, and manage the overall territory of their patient population which is typically geriatric (over the age of 65). About once a month this person will spend a weekend doing visits and will be compensated for the work done per visit on that weekend.
