Job Description
Insight Global is seeking a compassionate and detail‑oriented Care Manager in Woodland Hills, CA to support patients enrolled in our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role plays a critical part in coordinating care, maintaining consistent patient engagement, and ensuring patients receive the guidance and support needed to effectively manage chronic conditions. The ideal candidate is patient‑focused, organized, and comfortable working in a remote, technology‑driven healthcare environment.
In this role, you will spend your day engaging directly with patients via phone to provide ongoing support, education, and care coordination as part of our CCM and RPM programs. You will develop and maintain individualized care plans, monitor patient vitals and compliance data, and document all interactions accurately. You will collaborate closely with providers, nurses, and clinical leadership to ensure continuity of care, escalate concerns when patients are non‑compliant or show changes in condition, and serve as a consistent, empathetic point of contact for patients navigating chronic health challenges.
Key Responsibilities:
- Develop and manage individualized care plans for patients enrolled in CCM programs
- Conduct regular patient outreach via phone for RPM and CCM engagement
- Document patient interactions, vitals, and care updates
- Coordinate care with providers, nursing staff, and clinical leadership
- Monitor patient compliance and escalate concerns or non‑compliance as appropriate
- Deliver professional, empathetic, and patient‑centered communication at all times

Required Skills & Experience
Excellent written and verbal (telephone) communication skills
1–2 years of experience in healthcare, care coordination, or medical office administration
Strong organizational skills with the ability to multitask and manage patient caseloads
Comfort using technology, including EMR/EHR systems, spreadsheets, and connected health devices
Dependable, compassionate, and able to work both independently and as part of a team
Nice to Have Skills & Experience
Experience with Remote Patient Monitoring (RPM) programs
Familiarity with Chronic Care Management (CCM) workflows
Bilingual in Spanish preferred
