A Brief Overview
Hospice Nurse Case Manager Day-To-Day: you will visit 4-6 patients a day depending on location.
As a Hospice Nurse Case Manager, you will play a critical role in providing compassionate care to patients and their families during challenging times. Your responsibilities include:
Assessment and Collaboration:
Assessing patients and their families for physical, psychological, social, and spiritual needs.
Collaborating with patients, families, physicians, and the interdisciplinary group (IDG) to establish personalized care plans.
Implementing interventions and evaluating care effectiveness.
Effective Communication:
Initiating and maintaining communication with attending physicians, the Medical Director, IDG members, Team Directors, Team Managers, and other relevant agencies.
Coordinating optimal care and resource utilization for patients and families.
Accurate Documentation:
Documenting patient information according to established protocols.
Maintaining up-to-date patient records to reflect baseline, current, and ongoing status, including any changes.
On-Call and Admissions:
Participating in on-call duties and admissions assignments as directed.
Patient Education:
Providing education to patients and caregivers based on their specific knowledge needs and learning assessments.
Addressing patient and family conditions with empathy and expertise.
Coordination and Problem Resolution:
Coordinating care with the IDG.
Informing the Team Director and Team Manager of any unusual or potentially problematic patient/family issues.
Professional Development:
Participating in orientation and ongoing education programs.
Maintaining current professional licensure.
Compliance and Protocols:
Demonstrating familiarity with agency and state policies and protocols related to hospice and nursing services.
Ensuring compliance with internal process requirements and state/federal regulations.
Collaborative Meetings:
Participating in IDG meetings, facility patient conferences, and other patient/family meetings as coordinated with the IDG and/or Team Director.
Additional Responsibilities:
Fulfilling any other duties as assigned.

Education Qualifications
AA or Bachelor of Science in Nursing
Registered Nurse License in California
BLS - Basic Life Support
CPR

Experience
Hospice experience for 6 months to a year
EMR -- Care Point (homecare/homebase/point care)
