Job Description

An employer in the Southern California is seeking an RN Case Manager. This is a remote position, but individual must sit in Southern California area, willing to pick up equipment in Irvine. This individual will identify, coordinate, and provide levels of care to individuals, responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting.

Required Skills & Experience

-Current, unrestricted RN CA licensure

-3 or more years of Clinical RN experience

Strong inbound and outbound telephonic skills

Intermediate computer skills
