Job Description

Insight Global Health is looking for a remote RN Case Manager to work for a large healthcare company in the Irvine, CA area. Must sit in Southern California. In this position, you will be working with outpatient members, providers, and caregivers to facilitate intensive, comprehensive care management. These RNs will complete Health Risk Assessments, care plans, and monthly follow-ups with members. They will also make post-discharge calls within transition of care and will be involved in interdisciplinary meetings. The schedule is Monday to Friday, 8 AM to 5 PM. Strong communication skills are a must and knowing some EMR and MS Tools is a plus.

Required Skills & Experience

3+ years of experience as an RN Case Manager

Experience managing health care plans, facilitating post discharge planning and transition of care

Strong communication skills
