Job Description

This position supports the Regulatory Case Management team. The coordinator is responsible monitoring for Transition of Care events, Gather Medical Records, Initiate Member outreach to engage in Complex Case Management Program and coordinator with RN Case Manager. Coordinator is responsible for mailing letters, educational materials, and community resources for the Member. The coordinator is also responsible for facilitating/coordinating patient appointments, benefits, and other healthcare needs.

Required Skills & Experience

Minimum of 2 Years of HealthCare Experience



Knowledgeable in Medical Terminology



Computer Literacy, including Microsoft Office



Using databases specific to an organization to manage patient data



Critical Thinking, Time Management, Timeliness and dependability
