Job Description

A behavioral health company is looking for a REMOTE Clinical Case Specialist where they will conduct comprehensive clinical assessments and provide solution focused guidance, advocacy, and support on every call. Their responsibilities include:
- assessing clinical information provided and apply medical criteria as necessary
-coordinate behavior health advisor activities
- coordinate discharge planning needs with facility, health services providers, members and family.
-provide care coordination through referrals to physical health providers, social services, and disability services
-record and resolve member and provider inquiries and complaints
-triage calls from members in crises and assist to ensure safety and access to appropriate care
-maintain accurate and complete documentation in system

Required Skills & Experience

2+ years of experience in a behavior health role
Excellent attention to detail
Excellent written and verbal communication skills
Microsoft Office Experience
Master's Degree in similar field

Nice to Have Skills & Experience

Call Center Experience
Bilingual in Spanish
