Job Description
In this role, the analyst serves as a key leader in the implementation and optimization of Epic Healthy Planet for a mid-sized hospital or health system. They partner closely with population health leaders, care management teams, clinical stakeholders, quality departments, and IT leadership to design workflows that support value-based care initiatives, preventative care programs, and improved patient outcomes.
- Lead Healthy Planet workstreams throughout the implementation lifecycle, including planning, design, build, testing, go-live, and post-production support.
- Facilitate requirements-gathering sessions with operational and clinical leaders to translate business needs into Healthy Planet solutions.
- Configure and optimize registries, Compass Rose programs, care gap workflows, risk stratification tools, and quality measures.
- Support HEDIS and population health initiatives by building reporting tools, identifying care gaps, and monitoring quality metrics.
- Collaborate with analytics and quality teams to develop dashboards, Reporting Workbench reports, and population health insights.
- Coordinate system testing, workflow validation, end-user training, and go-live readiness activities.
- Act as the primary Healthy Planet subject matter expert for assigned projects, providing guidance to stakeholders and mentoring junior analysts when needed.

- Strong process documentation
- Experience supporting a mid-sized health system or hospital
