Job Description

Direct care coordination for patients who need hospice or home health care
Set up consultations with patients, families, and physicians
Evaluate patient eligibility for programs
Building relationships with physicians and case managers
Act as lead liaison for on-site clinical team

Required Skills & Experience

Active WA/Compact RN license
5+ years' experience as an Registered Nurse and/or Nurse Supervisor with lead responsibilities
Personal Vehicle
Experience in hospice, home health, and/or related specialty
