Job Description

Insight Global is looking for a Remote Licensed Practical Nurse to support a reputable Healthcare client. This individual is responsible for care coordination of members who are experiencing significant change in health status, which has resulted in the necessity of an Emergency Department visit, inpatient, skilled nursing, or rehabilitative stay. This individual ensures their designated members experience a seamless transition to their next care setting and facilitates post-discharge goal attainment. Examples include post-discharge questionnaires, ensuring timely physician/specialist follow-up, comprehensive and customized discharge planning, medication protocol, benefits education, post-discharge lifestyle plans, and reoccurring cadences to facilitate focused outreach, mitigate readmission risks, and support the member in achieving their personal optimal level of wellness within the confines of their disease processes. This LPN will be expected to conduct the following: Outreach cadence calls and post-discharge questionnaires, weekly and daily reporting, member progress evaluations to determine member's ability to function in the community and facilitate interdisciplinary care team meetings. Required Skills & Experience

3+ years of experience as an LPN (ideally between 3-10)

Active LPN license for state of residence

Experience planning patient discharge (set up home care, meal planning, delivery of health supplies, experience setting up community resources, etc.)

Prior experience working on Insurance Payer side (supporting patient care to be discharged for insurance providers)

Prior experience working in a fully remote environment.

Computer literacy (outlook and MS Teams, electronic documentation/EMR experience)

Ability to multi-task, prioritize and adapt to the fast-paced environment while providing exceptional care

Strong communication skills and work ethic

Nice to Have Skills & Experience

Compact License

Experience within home health or long-term patient care setting.

Understanding of HRAs or health risk assessments
