Job Description

Insight Global is looking for a Remote Licensed Practical Nurse to support a reputable Healthcare client. This individual is responsible for care coordination of members who are experiencing significant change in health status, which has resulted in the necessity of an Emergency Department visit, inpatient, skilled nursing, or rehabilitative stay. This individual ensures their designated members experience a seamless transition to their next care setting and facilitates post-discharge goal attainment. Examples include post-discharge questionnaires, comprehensive and customized discharge planning, medication protocol, benefits education, post-discharge lifestyle plans, and reoccurring cadences to achieve optimal level of wellness. This Transition of Care Coach (LPN) will be expected to conduct the following: Outreach cadence calls and post-discharge questionnaires, weekly and daily reporting, member progress evaluations to determine member's ability to function in the community and facilitate interdisciplinary care team meetings. We are seeking clinical professionals devoted to providing the highest level of care to their community. This Transition of Care Coach (LPN) will be provided with extensive onboarding and training in order to meet the needs of the organization and its members.

Required Skills & Experience

2+ years of experience as an LPN

Active and unencumbered LPN license for state of residence

Experience with discharge planning and post-discharge care coordination

Ability to multi-task, prioritize and adapt to the fast-paced environment while providing exceptional care

Strong communication skills and work ethic

High school diploma/GED or higher education
