Insight Global is seeking multiple Support Coordinator for a non-profit organization located in Philadelphia, PA. As a Support Coordinator, you will manage a caseload of individuals with Intellectual or developmental disabilities to help provide an over view of support. You will help connect them to the communities services, benefits and manage their ISP or Individual Support Plan. The first 3 weeks of this position is orientation/training (paid) which is hybrid. After training, you will enter the hybrid role of a Support Coordinator. You may get a balance to complete reports from home but you will be expected to be out in the field and travelling to your caseload of individuals on a daily basis. Travelling to and from clients will be reimbursed at the federal rate for gas mileage. The Supports Coordinator will engage individuals and their families in the development of plans that assure the needs and wishes of the consumers are addressed and life opportunities expanded. Supports Coordinators assist in the choice of services and supports, coordinate the provision of services and supports, and monitor the delivery of services and supports. This employee will promote individuality, choice and control. Experience in a Human Services or Social Work position
Documentation in a neat and timely manner
Drivers License and Personal vehicle
Bachelor's degree from an accredited college or university with at least 12 college credits in Social Science
Excellent communication/writing skills to interface effectively with all County Personnel, Program Coordinators, Contract Coordinators, Staff Members, Contracted Therapists, and other agencies
Ability to work in a high-pressured job situation and adhere to strict time guidelines
Ability to understand and accept the needs and rights of others and to work with individuals who are physically challenged, emotionally troubled, or economically disadvantaged.
