* Serves as a community care nurse or care manager for a defined panel of PACE participants that includes participants requiring a broad spectrum of care who reside in a variety of environments and may reside in a wide geographic area.
* Applies appropriate nursing theory and research to clinical practice.
* Performs comprehensive nursing assessment, consistently using appropriate clinical thinking skills and nursing judgment to develop, refine and evaluate an evidence-based nursing plan of care that reflects participant priorities, as part of the interdisciplinary plan of care, that is appropriate to a community setting and across the continuum of care.
* Performs and delegates procedures and treatments according to accepted clinical program standards.
