We are seeking an experienced Nurse Practitioner to join our Risk Adjustment team, specializing in end-stage kidney disease (ESKD) care. You will manage cases for patients with kidney disease, focusing on education, medical history, medications, support systems, lifestyle changes, and behavior modifications to reduce future medical costs through early identification and support for dialysis and transplant needs.
You will collaborate with nephrologists for rounding and annual assessment visits in dialysis centers, with travel to centers and practice offices. Telehealth and Home visits may be required as need (10-20% of the time.) All Mileage is reimbursed.
