Insight Global is seeking 40 Home Health Nurse Assessors to join a healthcare client in New York. These Nurses will operate on a hybrid model (3 days in person patient visits, 2 days remote patient intakes). They will be responsible for conducting initial assessments and determining qualification for long-term home care. Assessments of the patient and their environment will be thorough, lasting 2-2.5 hours each. For the first ~2 weeks, RNs will attend remote training sessions via Zoom from 8:30-5 PM. From there, RNs will be paired with Preceptors and will observe them conducting assessments for 1-2 days. The Preceptor will then observe the RN conducting assessments and provide reviews and feedback. Once training is complete, expectation will be 2 in person assessments during field days and 3 telehealth assessments during virtual days.

22 months of experience in a home health setting within the past 15 years (does not need to be experience providing long term care; any form of interfacing with a patient in the home will work)
Some experience conducting patient intakes/assessments
Computer literacy (Outlook, Zoom, MS Teams, screenshots, attachments, typing 35 words per minute)
ASN or BSN
BLS Certification from an accredited program
Unencumbered NY State license
Experience utilizing an online patient assessment system; Uniform Assessment System (UAS) preferred
Knowledge of NYIAP program
