Job Description
A client of Insight Global Health is looking for a Non-Clinical Care Manager for a remote 6-month contract-to-hire position. This is a 95% remote position. Some travel to office meetings could be required but will be infrequent. This individual will carry out the following responsibilities: Developing proactive care plans to address member health issues and enhance future wellness outcomes, using clinical tools and resources to conduct evaluations and questionnaires regarding members' needs and benefits, applying clinical judgement to incorporate strategies that will reduce risk factors and barriers in members' lives, conducting assessments based off information provided from various sources, collaborating with supervisors, interdisciplinary teams and key stakeholders involved in member healthcare plans, etc. This Care Manager will be expected to utilize case management processes in compliance with regulatory and company policies to ensure maximum member engagement.
Required Skills & Experience
Minimum 3+ years health related field -- ideally in a home health, assisted living, long term care planning, psych, virtual/remote care, or payer/insurance care management environment
Proven care coordination/patient care management experience, while partnering with interdisciplinary team members to develop plans and align on goals
Ability to develop care plans using resources like member's condition documentation, HRA content, etc.
Bachelor's degree
Nice to Have Skills & Experience
CRC, CDMS, CRRN, COHN, or CCM certification
Bilingual (English/Spanish; English/Creole)
Medicare and Medicaid experience
Managed care experience
Experience working with geriatric special needs, behavioral health and disable population
Knowledge of assessment, screenings, and care planning
Bachelor's Degree
