Job Description

3 month contract with high chance of extension. Must sit in Southern California area - remote but must be comfortable with coming into the Orange County office if laptop breaks down/circumstances arise.

The coordinator is responsible monitoring for Transition of Care events, Gather Medical Records, Initiate Member outreach to engage in Complex Case Management Program and coordinator with RN Case Manager. Coordinator is responsible for mailing letters, educational materials, and community resources for the Member. The coordinator is also responsible for facilitating/coordinating patient appointments, benefits, and other healthcare needs.

Required Skills & Experience

Minimum of 2 Years of strong Customer Service Experience in a Healthcare setting

Knowledgeable in Medical Terminology

Computer Literacy, including Microsoft Office

Using databases specific to an organization to manage patient data

Critical Thinking, Time Management, Timeliness and dependability

Strong communication/customer service skills
