Job Description

An employee in the Southern CA area is seeking an RN Case Manager. This position is fully remote, but candidate must sit in Southern California area. Must have unrestricted CA RN License. The RN Case Manager will help identify, coordinate, or provide appropriate levels of care for patients. Responsible for clinical operations and medical management activities across the continuum of care (assessing, planning, implementing, coordinating, monitoring and evaluating). This includes case management, coordination of care, and medical management consulting.

Required Skills & Experience

-Current, unrestricted CA RN licensure

-3 or more years of Clinical RN experience, resulting in excellent clinical skills and judgment

-Strong inbound and outbound telephonic skills

-Intermediate computer skills (Microsoft Word, Outlook and Internet) with the ability to navigate a Windows environment and the ability to create, edit, save and send documents utilizing Microsoft Word.
