The LVN Care Manager will support the Transitions of Care and Preventative care programs. The LVN Care Manager can contribute to the assessment of health status through gathering and recording of assessment data, observing, and monitoring and reporting of changes in condition to the supervising RN. Conduct interviews to understand cause and effect, gather or review health history for clinical symptoms, and determine health needs. The LVN Care Manager collaborates with the RN Care Manager on managing a successful transition from hospital to home. The LVN Care manager in collaboration with the RN Care Manager is responsible on completing timely post discharge patient outreaches to identify, develop, implement, and assist on interventions for high-risk patients. The LVN Care manager in collaboration with the RN Care Manager supports and manages the post-discharge needs to reduce hospital readmission, improve patient care engagement, and improve patient experience.

2-3 years of recent experience working in a Utilization Management LVN position
CA LVN license
Needs to be VERY strong with prior authorizations (no inpatient) since bulk of work will be focused on this
Experience with Medicare, MediCAL, commercial guidelines, etc
Prefers someone who has worked in a remote setting/tech savvy and can hit the ground running
Experience with DMs, hospital procedures, reviews and denials
