Job Description
The Liaison Nurse plays a vital role in supporting patients during the hospital discharge process by reviewing administrative paperwork, checking PICC and central lines for proper placement and function, and ensuring patients fully understand how to care for their central lines at home. This nurse will typically see about three patients per day, allowing for thorough education and personalized attention. The position offers a flexible schedule and requires strong communication skills to coordinate effectively with both patients and the healthcare team, ensuring a safe and smooth transition from hospital to home care.
