Job Description
The Manager works closely with BPP leadership, collaborating with the nursing Manager of Ambulatory Care Coordination, the Manager of Operations of the commercial plans, the clinical educator, and the quality specialist to develop strategies and implement best practices and workflows that support BPP contractual arrangements for both socially and medically complex patients within our contracts.

Required Skills & Experience
· Master’s degree in social work (MSW) – required
· 5+ years’ experience minimum (strong preference for 7+)
· ACO Experience ( Accountable Care Organization ) or Value Based Contracting Experience
· Experience building and developing programs along with program evaluation
· Ambulatory Experience
Nice to Have Skills & Experience
· Geriatric or Pediatric Experience
· Payer Background ( Humana, Aetna, UHG, Optum )
· Epic Experience
