Position Overview
We are seeking a Physical Therapist (PT) to join our team. In this role, you’ll use your clinical expertise to evaluate patients, provide education, and deliver therapy interventions as outlined in the home health plan of care. You’ll collaborate closely with an interdisciplinary team to support strong patient outcomes.
Schedule: PRN visits, Monday–Friday
Essential Job Functions
Conduct initial and ongoing assessments of patient mobility, strength, and overall function
Develop and implement individualized plans of care based on physician orders
Deliver physical therapy services and adjust treatment plans based on patient progress
Educate patients and caregivers on home exercise programs and ongoing care
Coordinate care with physicians, nurses, and other healthcare team members
Complete accurate, timely documentation in compliance with company and regulatory standards
Support discharge planning and ensure continuity of care
Communicate effectively with patients, families, and internal teams
Supervise PTAs and HHAs as applicable
Complete OASIS assessments (where applicable) and follow all policies, regulations, and care standards

Required Skills & Experience
Requirements:
Degree in Physical Therapy from an accredited university/program.
Current, unrestricted state license as a Physical Therapist in the state of practice
Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
