Job Description

Insight Global is seeking a Care Coordinator at a behavioral health organization to complete comprehensive client assessments, develop individualized service plans in collaboration with clients and therapists, and connect clients and their families to appropriate community resources. This role will advocate for clients by coordinating with various agencies and programs, conduct face-to-face visits to monitor progress, and maintain accurate, detailed documentation through daily progress notes. The Care Coordinator will also support program goals by working closely with law enforcement and county jails while ensuring services align with each client’s overall care plan.

Required Skills & Experience

Must-Haves:
• Bachelor's Degree from an accredited university with major course work in the areas of healthcare, psychology, or social work.
• 1-year experience in Care Management with primary care, SMI, SUD or similar populations required.
• Must have a current Florida Driver's License with a clean driving record while meeting company insurance requirements.
• Strong interpersonal skills with the ability to engage effectively with patients
• Open-minded and adaptable
• Comfortable with a national and state background check (DCF finger print clearance (level 3)
Team-oriented while able to work independently

Nice to Have Skills & Experience

Criminal justice experience
