Job Description
A client in the Brooksville, FL area is looking for an LPN to join their team for a 3-month contract to hire. The position involves managing an active nursing appointment schedule to assist Providers in monitoring members’ healthcare needs and collaborating with Care Managers/Providers to ensure appropriate follow-up for post-hospitalization or emergency room visits. The nurse comprehensively screens patients for care opportunities, works with treating physicians to develop individualized care plans, and provides condition-specific education, anticipatory guidance, and care coordination under Provider direction. Additional responsibilities include delivering health teaching, direct nursing care, comfort measures, and emotional support to patients, as well as performing selected IV therapy and administering prescribed treatments per Provider orders.

Required Skills & Experience
• Valid Florida LPN license
• BLS Certification
• IV certification or the ability to obtain within 90 days of employment
• High school graduate, GED or equivalent
Nice to Have Skills & Experience
• Experience with Eclinical Works (EMR)
• 2+ years of experience in Med-Surg, ER, ICU, Acute Care
• Experience with electronic health records (EHR)
• Experience in MS Office
