Job Description

Our Care Managers are frontline advocates for members who cannot advocate for themselves. They are responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. You can have life-changing impact on our members who are enrolled in Medicare and Medicaid and present with a wide range of complex health and social challenges. A client of Insight Global Health is looking for a Non-Clinical Care Manager for a remote 6-month contract-to-hire position. This is a 95% remote position. Some travel to office meetings could be required but will be infrequent. This individual will carry out the following responsibilities: Developing proactive care plans to address member health issues and enhance future wellness outcomes, using clinical tools and resources to conduct evaluations and questionnaires regarding members' needs and benefits, applying clinical judgement to incorporate strategies that will reduce risk factors and barriers in members' lives, conducting assessments based off information provided from various sources, collaborating with supervisors, interdisciplinary teams and key stakeholders involved in member healthcare plans, etc. This Care Manager will be expected to utilize case management processes in compliance with regulatory and company policies to ensure maximum member engagement.

Required Skills & Experience

Must Haves:

* Minimum 3+ years health related field -- home health, care coordination, long term care planning, psych, or payer/insurance care management is ideal experience





Plusses:

* CRC, CDMS, CRRN, COHN, or CCM certification

* Bilingual (English/Spanish; English/Creole)

* Medicare and Medicaid experience

* Managed care experience

* Experience working with geriatric special needs, behavioral health and disable population

* Knowledge of assessment, screenings, and care planning

* Bachelor's Degree
