Job Description
The TAY Case Manager & Navigation Specialist works directly with young adults to assess needs, develop individualized service plans, and connect participants with housing opportunities and community resources. On a daily basis, this person meets regularly with youth to monitor progress, address barriers to stability, provide ongoing guidance and emotional support, and help develop achievable goals related to housing, employment, education, life skills, and self-sufficiency.
This role assists youth with navigating housing resources, completing applications, gathering required documentation, and securing permanent housing placements. The Case Manager also provides crisis intervention when needed, conducts home visits after housing placement, and helps participants strengthen positive support networks through family connections, mentorship opportunities, and community engagement. In addition, the position partners closely with internal teams and community organizations to coordinate services, develop life skills programming, facilitate activities and workshops, maintain accurate case documentation, and ensure compliance with program reporting requirements.

Required Skills & Experience
Bachelor's degree in Social Work, Psychology, Human Services, Sociology, or a related field
2+ years of experience working with transition-age youth, homeless populations, or other vulnerable communities
Experience providing direct case management and service planning
Experience with crisis intervention, de-escalation, and resource navigation
Strong understanding of the challenges facing homeless and at-risk youth
Ability to build trust and establish positive relationships with young adults
Nice to Have Skills & Experience
Master's degree in Social Work, Counseling, Psychology, or a related field
Experience working within transitional housing, rapid rehousing, or supportive housing programs
Knowledge of Positive Youth Development and youth-centered service models
Familiarity with Trauma-Informed Care, Housing First, Motivational Interviewing, and Harm Reduction practices
Experience assisting youth with independent living skills development
Experience conducting home visits and housing stabilization services
Bilingual in English and Spanish
