Job Description
This program offers resources to patients recently discharged from an inpatient behavioral health unit. Eligible patients have a mental health diagnosis. The clinician will conduct a consultation with patients to evaluate their mental health and identify any unmet needs. Clinicians are not conducting therapy, providing diagnoses, or managing medications. The program you work on is subject to change depending on business needs.

1 - Mon-Fri 8am-4:30pm PST
1 - Mon-Fri 8:30am-5pm PST

Required Skills & Experience
● Education and Licensure:
○ Master’s degree in Social Work (MSW)
● Licensing (U.S.):
○ Must hold a valid state license to practice, such as:
■ LCSW – Licensed Clinical Social Worker
● 1-2 years of social work experience, especially in:
○ Behavioral Health (inpatient or outpatient)
○ Medical/Hospital setting
○ Community Mental Health
○ Community Services
○ Case Management
● Ability to speak and write effectively
● Ability to solve problems and identify solutions
● Ability to demonstrate customer service skills in interactions with all patients,
families, and staff including in high-volume and stressful situations
Nice to Have Skills & Experience
● Experience providing remote or telehealth services is a plus but not required
