Job Description

A startup healthcare client in the San Francisco area is looking to hire a remote LVN or LPNs with a multi-state compact license. You will be in charge of meeting with around 10-15 patients a day for 20 minutes to go over the patients care plan and medications, followed by strict documentation for Medicare compliance. He or she will have weekly check ins with the supervising RN. All LVN/LPNs will start with outreach/enrollment for 2–4 months before transitioning into care. The timing depends on our total patient count at that time of starting.

Additional Responsibilities:

• Provide ongoing care for patients with neurological conditions through monthly check-ins
• Assess changes in patient baselines and recommend patient-specific care resources
• Communicate with caregivers on care improvements and next steps
• Coordinate care: help schedule referrals and appointments, notify clinics of medication refill needs, send medication reminders
• Find and vet regional and disease-specific resources for patients (exercise, diet, sleep, social engagement)
• Collaborate with our AI engineers by providing weekly feedback on our care platform. Your clinical perspective directly informs what we build.

Salary: $60,000-$70,000

Required Skills & Experience

- Active LVN/LPN license (+ Multi-state compact)
- Clinical experience working with patients with neurological conditions (Alzheimer’s, Parkinson’s, MS, etc.). Work in clinics, hospitals, or as a care manager is preferred.
- Ability to clearly explain neurological conditions, symptoms, disease progression, modifiable risk factors, and medical/non-medical interventions.
- Organized and able to learn quickly on the job while maintaining systems
- CCM (Chronic Care Management) experience
