LVN Case Manager

MUST SIT IN SOCAL. Remote position. An employer in the Southern CA area is seeking an LVN Case Manager. The LVN will be managing a daily queue of patients recently discharged from an Inpatient setting admission that are deemed to be higher risk patients due to age or dx. Nurses will call and engage patients and review status of health and DC needs, assure all needs are met appts made etc.. Nurses will schedule calls weekly to follow patients for 30 days post discharge and stay connect goal is to prevent readmissions.
