Insight Global Health is looking for a remote RN Case Manager to work for a large healthcare company of ours in the San Bernadino area. In this position, you will be working with outpatient members, ACO, disease management, providers, and caregivers to facilitate intensive, comprehensive care management, and increased efficient utilization of services. The RN will work with chronic or catastrophic cases through the care management process according to program guidelines. They will provide healthcare resources, counseling, and on-demand information to patients who have a specific chronic condition, as previously identified by the ambulatory case manager. The RN will coordinate care needs with the patient's primary care provider and will work Monday to Friday, 8 AM to 5 PM, and need to be local to the Ontario or Chatsworth regions. Occasional office work in Chatsworth is required. Strong communication skills and knowing some EMR and MS Tools is also a must.
- 2+ years of Case Management, specifically in ACO & disease management
- Unrestricted RN CA license
- Excellent computer skills
