Job Description
An employer is looking for a Staff Specialty Pharmacist for a Specialist Pharmacist in Gilbert, Arizona. The client works closely with
patients, caregivers, healthcare providers, and payers to ensure a seamless treatment journey (primarily rare diseases). The Staff Pharmacist contributes to both the clinical and operational aspects of specialty pharmacy services, including prescription evaluation, patient education, therapy monitoring, and interdisciplinary collaboration. The role serves as a key clinical resource for patients, caregivers, healthcare providers, and internal teams to help drive positive treatment outcomes.
Primary Responsibilities:
- Provide clinical support, education, and ongoing guidance to patients and caregivers throughout therapy.
- Conduct patient assessments to identify adherence concerns, therapy risks, and care needs.
- Counsel patients on medication use, expectations, side effects, and treatment goals.
- Monitor therapy outcomes, adherence, and patient-reported concerns through routine follow-up.
- Collaborate with providers, care teams, and payers to improve medication access and continuity of care.
- Support the development of individualized care plans that meet quality and accreditation standards.
- Identify and resolve medication-related issues to promote safe and effective therapy.
- Contribute to quality improvement initiatives through clinical documentation and outcomes review.
- Serve as a clinical resource for pharmacy staff and assist with escalated patient or operational issues.
- Ensure compliance with all applicable regulations, accreditation requirements, and pharmacy best practices.
Hours: Mon-Friday 8:30AM-5PM
Rotating on-call weekend support for overflow (Monthly)

Required Skills & Experience
- Minimum 1 year of professional experience as a pharmacist, or equivalent training and
experience
- Specialty Pharmacy experience preferred (Open to candidates with Postgraduate Year One (PGY1) Specialty Pharmacy Residency)
- Doctor of Pharmacy required
- Active Pharmacist license in good standing issued by the Arizona State Board of Pharmacy
- Ideally looking for case management and/or polypharmacy care models
- Strong problem-solving skills and an out of the box thinker
Nice to Have Skills & Experience
- Active New York License
- URAC and ACHC specialty accreditation standards
- Certified Specialty Pharmacist (CSP)
